IOSHA to investigate man crushed by forklift

SOUTH BEND, Ind. (AP) - The Indiana Occupational Health and Safety Administration will investigate the death of a South Bend man crushed by a forklift.

St. Joseph County deputy coroner Chuck Hurley tells the South Bend Tribune that 47-year-old Michael Gorzkiewicz died of blunt force trauma to the chest. Hurley says an initial investigation showed that Gorzkiewicz was behind a forklift that was parked on an inclined ramp at a Steel Warehouse plant on Monday when the forklift rolled backward and crushed him between it and a truck.

South Bend police say the forklift was impounded.
