SHELBYVILLE, Ind. (WISH) - Another accident at the Shelbyville glass manufacturer where a worker died in 2010 is facing new scrutiny from state and local investigators, 24-Hour News 8 news partner The Shelbyville News reports .

The latest accident happened Monday. Shelbyville Fire Department was called to Pilkington, 300 Northridge Drive, on a report of a man stuck in a press at about 11:30 a.m. The man was flown to IU Health Methodist Hospital in Indianapolis, but no further information was available about him.

The Indiana Occupational and Safety Administration fined the plant $600,000 for violations over the summer after inspectors found several repeat or ignored violations. Pilkington is appealing some of the fines.

But now, Bob Dittmer, a spokesman for the state Department of Labor, told the newspaper that IOSHA will likely start another investigation.

Pilkington, which makes glass for the automotive industry, employs 350 at the Shelbyville plant.
