INDIANAPOLIS (WISH) - The Indianapolis Colts are teaming up with Anthem Blue Cross Blue Shield to honor everyday heroes for the fifth year.

The program exists to pay tribute to Hoosiers who may have received little or no recognition for the efforts they make to help others.

The 2012 "Anthem Angels…Honoring Everyday Heroes" program will recognize its first "angel" of the year during the Colts first home regular season game on Sunday, Sept. 16.

The Colts will accept nominations through Dec. 9 to honor civil servants who have made outstanding contributions above the call-of-duty- as “Anthem Angels,” according to a release.

A nomination form is available online .

One “Anthem Angel” will be selected before each 2012 Colts regular season home game.

Each person will receive four VIP Club Seat Tickets to the game, a feature story in the Colts game day magazine, and recognition on field at the game.
